Participant Information
1 · Activity Description
The participant will take part in a Youth Ocean Experience outing organized by My Clean Ocean (“the Organization”) on the date noted above: a supervised day trip aboard a professionally operated vessel with a licensed captain and crew. Activities may include being aboard a moving vessel at sea, observing marine wildlife, supervised hands-on boat activities, and being filmed and photographed for the Organization's storytelling programs.
2 · Acknowledgment and Assumption of Risk
I understand that ocean and boating activities carry inherent risks that cannot be eliminated even with reasonable care, including but not limited to: slips and falls on a moving deck, seasickness, sun and weather exposure, contact with marine life, equipment failure, and delayed access to medical care while at sea. Having read and understood the above, I voluntarily allow the participant to take part and I assume all such risks on the participant's behalf. Parent/guardian initials:
3 · Release of Liability and Indemnification
To the fullest extent permitted by law, I release, waive, and agree not to sue My Clean Ocean, its directors, officers, employees, volunteers, and agents, and the vessel owner/operator and crew (together, the “Released Parties”), from any and all claims, demands, or causes of action arising out of or related to the participant's involvement in the activity, including those alleging ordinary negligence of the Released Parties. This release does not extend to gross negligence, recklessness, or intentional misconduct. I further agree to indemnify and hold harmless the Released Parties from claims arising from the participant's own actions. Parent/guardian initials:
4 · Medical Authorization
If I cannot be reached in an emergency, I authorize the Organization and its representatives to obtain and consent to reasonable emergency medical treatment for the participant, and I accept financial responsibility for such treatment.
5 · Travel Consent
Some outings travel beyond United States waters into international or Mexican coastal waters and return the same day, without a scheduled land stop. Registered families are notified in advance when an outing will do so, including any travel-document requirements (such as a valid passport or passport card). For such outings, I consent to the participant traveling aboard the vessel and authorize the Organization's supervising adults to accompany the participant for the duration of the outing. 6 · Media Release
I grant My Clean Ocean an irrevocable, royalty-free, worldwide right to photograph, film, and record the participant during the activity, and to use, edit, publish, and distribute that material — including the participant's image, likeness, voice, and first name — in any media now known or later developed (including the Organization's website, social media, fundraising materials, grant reporting, and advertising), without compensation. I understand no personally identifying details beyond a first name will be published without separate consent.
